Easy and convenient: form for submitting request for public information

State Tax Service of Ukraine recalls that to simplify the procedure for making a written request for public information, it is possible to use the Form for submitting request for public information which is in the possession of the State Tax Service.

Specified Form and sample of its completion are posted on the official web portal of the State Tax Service in section “Public Information” – the Request Form https://tax.gov.ua/priymalnya-gromadyan/dostup-do-publichnoi-inform/formi-zapitu-na- informatsiyu / 429621.html.

E-mail address of the State Tax Service for requesting public information: publicinfo_dps@tax.gov.ua.

Please note that suggestions, applications, complaints of citizens, legal entities, individuals – entrepreneurs, self-employed individuals, as well as issues concerning the practical application of certain norms of tax legislation, clarification issues regarding taxpayer’s rights and obligations from the single contribution to compulsory state social insurance are not sent to the e-mail address publicinfo_dps@tax.gov.ua

Suggestions, applications, complaints are considered by the State Tax Service according to requirements of the Law of Ukraine “On Citizens’ Appeals” and such appeals from citizens are sent to the e-mail zvernennya_dps@tax.gov.ua  – in the form of scanned or photocopies of written appeals signed by the applicant indicating date and place of residence (registration). Citizen’s signature in the electronic application can be scanned or photographed. Electronic citizens’ appeals signed with the use of a qualified electronic signature are also processed.

Applications from legal entities and self-employed individuals are accepted to the e-mail address post@tax.gov.ua
