Where to start?

Submit a Registration card of individual in the form № 1DR, which is also application for registration in the State register of individuals – taxpayers.

Who submits documents?

Registration card in the form № 1DR is submitted by one of the parents (adoptive parents, adoptive parents, guardians) or a representative by the power of attorney for a minor child under 14 years old.

What documents are needed?

Child’s birth certificate.

If a certificate is not issued in Ukrainian, it is necessary to provide translation of such certificate into Ukrainian, certified according to legislation (original is returned upon presentation) and its copy.

Document certifying identity of one of the parents (adoptive parents, adoptive parents, guardians).

Additional documents (if necessary):

- If the parents or one of them were foreigners or stateless persons at the time of child’s birth, registration certificate of a citizen of Ukraine must be provided (returned upon presentation).

- To confirm place of residence (stay), it is possible to provide: extract from register of the territorial community, passport in the form of a booklet, temporary certificate of a citizen of Ukraine.

- To confirm status of internally displaced person, registration certificate of internally displaced person is provided.

Where to submit documents?

To any controlling authority in a person or electronically via E-cabinet or mobile application "My Tax Service".

How long does it take?

Registration of a person in the State register of individuals – taxpayers is carried out within 3 working days after application.

How to get a ready-made document?

Document certifying registration of a child in the State register of individuals – taxpayers (taxpayer card) can be issued to one of the parents (adoptive parents, adoptive parents, guardians) or representative by the power of attorney.
