Citizens who have changed their personal data (surname, first name, patronymic, residence place, etc.) are required to notify the tax authorities during one month.
It is possible to update personal data conveniently and without queues through the “payer's Electronic cabinet”.
Apply online based on the following documents:
- For citizens of Ukraine: passport (booklet or ID card).
- For children (under 14 years old): birth certificate.
- For foreigners: temporary or permanent residence permit in Ukraine in the form of ID card.
Step-by-step algorithm of actions in the Еlectronic cabinet:
1. Login and creation: Select the “Electronic cabinet for citizens” mode, item “Application for changes to the State register of individuals (5DR)”.
2. Filling in data:
- Indicate your Registration number of the taxpayer's account card and current personal information (last name, first name, patronymic, etc.).
- If there has been a change in the name, be sure to indicate previous data.
- Fill in information about the birth place and current address of the declared (registered) place of residence (stay). For internally displaced persons, a special field is provided for entering data about registered residence place according to the registration certificate of internally displaced person.
1. Documents: Specify requisites of the identity document (series, number, issuance date).
2. Choose the Taxpayer Service Center: Choose the Taxpayer Service Center convenient for you, where it is possible to receive a ready-made taxpayer card.
Annexes to the application
Scanned copies of documents are required for application submission. Copies must be in the PDF format or as images:
- For passport booklets – all completed pages (1–6 and registration).
- For the ID cards – both sides and residence certificate.
- If changing surname, first name, patronymic – certificate of marriage, divorce or change of name.
- In case of changes regarding minors (up to 14 years old) – the child’s birth certificate and document certifying identity of one of the parents (adoptive parent or guardian), certificate from the residence place, document confirming change in the child’s last name (if there is such a change).
Final stage
After filling in, click “Save”, sign document using a qualified electronic signature and send it to the State Tax Service. The system will automatically check correctness of the fields before sending.
